Well
Child Visits:
To keep your child healthy,
it is important to bring them in for
regular checkups. We recommend that
you call several weeks in advance so
you may choose the date that meets your
needs. |

Below
is our recommended schedule for Well
Child Exams:

Newborn initial visit 2 – 5 days
after discharge
• 8 days - 2 Weeks
• 2 Months
• 4 Months
• 6 Months
• 9 Months
• 12 Months
• 15 Months
• 18 Months
• 2 Years
• 3 Years
• 4 Years
• 5 Years
• Annually thereafter up to age
22 years (for older females Pap
if indicated) |

Colorado Required and Recommended Immunization
Schedule as approved by the
Advisory Committee on Immunization Practices,
The American Academy of Pediatrics and
the American Academy of Family Physicians. |

Sick
Child Visits
We make every effort to schedule appointments
for ill children on the day that you call.
If you have a medical emergency during
office hours, we will do our best to see
your child immediately or refer you to
the appropriate facility.
Two or more children: If you would like
us to see and examine more than one child,
please schedule both of them when you
make your appointment. Without an appointment,
we may not see a sick sibling right away,
and you may be asked to wait while the
provider sees a scheduled patient who
may be waiting. |
